CP Unlimited’s Day Habilitation programs and Article 16 Clinics for people with intellectual and developmental disabilities are offered in unique settings throughout New York City and Hudson Valley.
While each location provides different activities and options for local engagement and enrichment, all CP Day Habs provide top-level services, educational classes, and PT/OT/Speech/Mental Health therapies delivered by caring, patient, and dedicated staff. The Linda Buch Gherardi Center is a centrally-located day program for people with I/DD in Crown Heights, Brownsville, Prospect-Lefferts Gardens and the surrounding communities. Offering an array of flexible options and door-to-door transportation, the LBG Center is a place where the goal for every team member is to help each person achieve the maximum level of independence through training, education, and support. Clinical support in the areas of nursing, psychology, and physical, speech, and occupational therapies are provided daily.
The program operates Monday through Friday from 9:00 AM – 3:00 PM. Persons supported here are frequently dependent on wheelchairs for mobility and need close care for daily rituals.
This special building, formerly an ice cream factory (and still a sweet and joyous atmosphere), provides more than 65 persons supported each year a full calendar of enriching activities including art and music while taking advantage of Lincoln Terrace / Arthur S. Somers Park for leisure and nature. A unique gardening program lets participants grow herbs and then sell them to local residents, while a companion recycling program helps persons supported learn money management and celebrate together each month.

The Carmen Aquilone Day Program assists more than 80 people each year in a barrier-free environment, where individual choice and skills-based training are at the center of our comprehensive approach to support. We promote growth by providing money management, safety and community awareness, fire safety / evacuation, socialization, hygiene, and more.
In practice, this means healthy and safe food choices are paralleled with eating and drinking skills, while physical therapy sessions incorporate seated yoga poses for deepened range of motion and mindfulness.
The Carmen Aquilone setting also features a Senior Satellite Program, promoting the same services tuned to older individuals.

CP’s West Farms Center is a LEED-GOLD Certified setting, opened in 2012, providing leading supports and community offerings for close to 200 people with intellectual and developmental disabilities every year.
This full-service setting features a number of special offerings for attendees, including our pottery kiln, West Farms Photo Club, and Multi-Purpose Auditorium with a professional stage and A/V system for performances, games, and programs. Over in the Horticulture Room, attendees grow purple basil, thumbelina carrots, curly blue kale, microgreens, and more for a local hospitality company. For a break, persons supported head to the rooftop terrace for time in the sun and to safely gather for activities. There is even a space for companion animals and support pets.
For The Bronx community, West Farms offers a Grab n’ Go Café, where the people we support work and utilize money management skills, offering light breakfast, coffee, lunch, and snacks. West Farms also features a Metro Community Health Centers clinic, offering medical, dental, psychological, neurological, and women’s health services for area residents. One additional opportunity unique to West Farms is our After School Supplemental, which many attendees utilize to extend their family care and education.
West Farms Photo Works Club
One of the most visible groups in the I/DD world, the West Farms Photo Works in the Bronx help persons with disabilities express themselves through photography while promoting independence, creativity, and personal resilience.
Participant photography has been featured in galleries throughout New York City as special exhibitions and as inclusions in collective exhibits. See a news article on the recent exhibition at West Farms in the Bronx.
Ceramic Studio at West Farms
People supported enjoy the educational, social, and emotional benefits of working with clay at CP Unlimited’s ceramic studio in The Bronx.
Participants create objects d’art like bowls, mugs, cups or free form sculptures using a variety of techniques and fired in CP’s kiln—one of the only kilns in the entire borough, and we believe the only one available to people with I/DD— which turns them into a piece ready for a final glazing by the artist.

A true oasis for the Queens community of people with I/DD and their families, CP’s Long Island City program supports more than 70 people with mobility therapies, activities, outings, educational programming, and daily enrichment.
Attendees enjoy time in Astoria Park, and love their experiences at Astoria 360 for bowling and other fun, with cultural exposure at the Museum of the Moving Image, the Noguchi Museum, and other local institutions. A monthly recycling program simultaneously teaches money management and environmental conservation, and is always greatly anticipated by program participants.

The Cora Hoffman Center stands out among CP Unlimited’s Day Habilitation network. It is situated in an area of Staten Island with less access to support, making it one of the more necessary as well as one of our most diverse in terms of what is offered on site, including a Day Habilitation Program, Vocational Supports, Wheelchair Clinics, Article 16 Services, and a co-located Metro Community Health Centers offering all manner of medical, dental, and psychological services.
The Cora Hoffman Center is, therefore, a beacon for nearly 100 persons supported who enjoy unique options including the M.O.V.E. (Mobility Opportunities via Education/Experience) program and our SMILE Farms Greenhouses, alongside regular visits from Wild Zoo!, trips to the Ferryhawks games, music, games, and holiday outings.
Smile Farms at Cora Hoffman Center
In partnership with Smile Farms, the Agency supports two full-time employees and a rotating community of Day Hab participants to help plant, harvest, and distribute the crops from on-site greenhouses. They also work at local events to share and sell produce and hot sauces, offering opportunities for skill building and visibility alongside funding as the proceeds from the sauces help fund programs for people with I/DD.

When CP Unlimited expanded to the Hudson Valley in 2018, we promised Putnam and Dutchess County leaders that our program would be a leading source of inspiration and education. Today, we have fulfilled that pledge to become a trusted community partner across the region.
The Mt. Ebo team are building bridges in the local community with visits from the SPCA and local musicians, visiting the local library, bringing in local authors for readings, and developing a thriving partnership with the local Rotary, Elks, and other groups.
Participants also enjoy opportunities to attend rallies in Albany and speak with elected representatives, while broadening their personal horizons through activities ranging from art, sports, exercise and movement, and weekly karaoke parties. The Agency also provides glittering opportunities for connection at our Spring Fling and Fall Formal gala events. On the horizon are plans for helping farm animals at a local farm/treatment center, developing a swim program, and creating more volunteer opportunities to help our shared communities.
