Constructive Partnerships provides comprehensive residential services throughout New York City, Yonkers, and Hudson Valley for people with intellectual/developmental disabilities. Our residential programs create a caring environment while promoting independence and individual choice.
Persons receiving support are provided person-centered care from a diverse and highly trained clinical team which includes a Medical Director alongside people overseeing Clinical Services, Nursing, Rehabilitation, and Nutrition, as well as certified and licensed OT, PT, Speech (Article 16) professionals, alongside a strong Behavior Support team.
The Agency operates 2 types of residential programs: Individualized Residential Alternatives (IRA), either full houses or individual apartments offering 24-hour staff support and supervision, and Intermediate Care Facilities (ICF), which are homes providing health related care and services to people who require services beyond what is offered in IRA settings.
In addition, CP Unlimited provides OPWDD subsidy/ISS (Independent Living Support Services) for 86 people living in 80 independent homes across New York City.
Via Creative LifeStyles, CPU also is responsible for nine residences in the Bronx with 41 residents, and for 25 individuals living in their own homes and who receive Community Habilitation services through this provider.
CP Unlimited embraces patient-centered care and participates in the Centers for Medicare & Medicaid Services’ aimed at improving health outcomes, reducing avoidable ER visits and reducing hospitalization. To that end, CP offers telemedicine services to keep people in the comfort of their homes while caring for non-emergency acute medical complaints. This commitment to spearheading technology and education to maintain top-levels of care is also highlighted by our decades-long participation in the Mount Sinai Palliative Care training program and ongoing internal training and certification regimen.
In 2024, the organization supported 550 people living in IRAs and 132 people in ICFs across 120 residential locations throughout New York City, Yonkers, and the Hudson Valley.
